Burning questions arise when cow’s milk allergy Burning questions arise when cow’s milk allergy (CMA) is suspected in a fully breastfed baby. These and other CMA concerns are addressed in this fascinating 2025 review article! There’s a lot we still don’t know, but its summary of we DO know is awesome. * Measurable concentrations of β-LG in breastmilk were detected UP TO 7 days after maternal intake of cow’s milk

* Studies aimed at estimating the peak allergen levels following maternal ingestion of cow’s milk have shown that the highest concentrations occur between 4 and 6 h after consumption 

* It is noteworthy that it has been demonstrated that the detection of β-LG in the breast milk of exclusively breastfed infants with atopic dermatitis is associated with the measurement of β-LG in infant sera. This provides evidence that β-LG contained in breast milk is transferred into the circulation of infants after maternal breast milk intake 

* Some researchers found no correlation between maternal allergies and β-LG levels in breast milk, while others observed elevated β-LG levels in mothers with atopic conditions 

*  Symptoms appear a few minutes after ingestion; in infants, they usually occur immediately after breastfeeding. IgE-mediated CMA is rare in exclusively breastfed infants, but some case reports have documented allergic reactions due to cow’s milk protein transfer through human milk

Sadly, many families are overwhelmed or don't have the support they need to help transition their babies to the breast. The process usually takes a few weeks to about 2 months. Again, with expert help!
A new study found these babies do not have high exclusive breastfeeding rates - only 32% of the babies.
This new study says, "Babies with Down syndrome (DS) present physical characteristics that can make breastfeeding difficult, including hypotonia of the muscles of the oral cavity and pharynx which can create problems with sucking and swallowing. Breastfeeding has proven beneficial for babies, and it is recommended to be exclusive for the first 6 months. However, babies born with DS have a higher probability of not being breastfed." "Results: The mothers reported that 98% of their babies had received mother's milk at least once and that 64% had received infant formula in the first three days of life. The median duration of breastfeeding was three months; 32.4% of the babies were exclusively breastfed for 6 months. Risk factors for not being breastfed were problems with sucking, swallowing, and not providing feeding on demand, as well as improper technique." While at 6 months, the iron group had higher hemoglobin levels, by 1 year there was no difference AND the iron group gained less weight and had less growth in head circumference. Does expressing milk at the end of pregnancy contr Does expressing milk at the end of pregnancy contribute to faster milk increase after birth? No, according to this 2025 study of mothers expecting babies with congenital anomalies. Its researchers taught the study mothers hand expression before birth, they practiced it, and it did not speed milk increase—aka coming to volume or secretory activation--after delivery. We know that learning hand expression before birth has other benefits. For example, it can boost confidence and reduce formula supplementation during the hospital stay. But it does not seem to affect how quickly milk production ramps up after delivery. Research shows just how important dads & partners Research shows just how important dads & partners are to breastfeeding success. Though they may feel like they aren’t doing much when a parent is exclusively breastfeeding, the studies tell us the opposite!  In a 2022 study looking at black families, the researchers found, “Participants who breastfed for at least 6 months largely attributed their breastfeeding success to the support received from their partners.” (Dogan et al, 2022). This trend of partner support was seen across all research regardless of geographical area, race, or income level. A 2022 review had the same results. “This review shows that a father’s support of breastfeeding improves breastfeeding outcomes. Including fathers in the breastfeeding process and ensuring their active participation increase breastfeeding rates.” (Koksal et al, 2022)

Bottom line - our dads/partners matter when it comes to breastfeeding success!

Background: While breastfeeding benefits early child neurocognition, its influences into adolescence, a period of intense brain remodeling and heightened mental health risk, remain unclear.

Methods: Breastfeeding and neurocognitive longitudinal associations were explored over a two-year period in the Adolescent Brain Cognitive Development (ABCD) Study® (nbaseline = 5098, ages 9-10, 49% female; nfollow-up = 3810, ages 11-12, 48% female). Breastfeeding duration was reported as never breastfed (15.8%), 1-6 months (34.6%), 7-12 months (26.4%), and >12 months (23.1%). MRI-derived estimates of cortical thickness, surface area, and cortical myelin were calculated across 148 brain regions alongside fluid cognition measures. Linear mixed-effects models tested the influence of breastfeeding duration and its interaction with age on neurocognitive outcomes. Significant cortical thickness and surface area associations were explored for cortical myelin differences. Parallel mediation analyses examined whether cortical features mediated the breastfeeding-fluid cognition relationship.

Results: Breastfeeding duration was positively associated with cortical thickness (31 regions), surface area (45 regions), and fluid cognition (all p values < 0.05), and with greater cortical myelin in four regions and increases by follow-up in 12 regions (all p values < 0.05). Surface area mediated the breastfeeding-fluid cognition link (β = 0.008, CIboot95% = 0.005, 0.012).

Conclusions: These findings emphasize the importance of extending breastfeeding practices for optimal adolescent neurocognition.
